You are  hereby requested to kindly submit completely filled up forms of applications  for CGHS card of beneficiaries from your Office  and send to this Office latest by 18.07.2013 positively  for onward submission to CO. It  may kindly be intimated to the beneficiaries to mention “Bhubaneswar” including their full residential address in the application form The application form is enclosed herewith for necessary action at your end.

 This may be treated as most urgent.
